For those members that would like to participate in the UPM CBA Survey for January 2019, but have not yet created a Member Account on the UPM Website, you have the option of completing the survey using the PDF Form version.
Below is the UPM Collective Bargaining Survey as an intercative PDF form. Please complete the survey to advise the UPM Collective Bargaining Team and the UPM Executive Council your priorities for the upcoming Collective Bargaining sessions with the district for our the 2020-2022 CBA.

After you have completed the form, SAVE it to your computer, then PRINT a hard copy to submit to UPM.
